Q1: How do I obtain my proof of immunization?
You can access your vaccine records two ways:
- Online through MyHealth Records. This process can take a couple of weeks.
- Paper record received at your vaccination appointment. If you lost your paper copy, please contact your immunization provider.
- Pharmacy or physician's office where you received your vaccination.
- AHS Clinic - call 811 to request a copy of your record mailed to you.
Q2: What is the criteria for receiving an accommodation?
Individuals applying for employment and Alberta Precision Laboratories (APL) employees who are unable to be immunized due to a medical reason, or for another protected ground under the Alberta Human Rights Act will be reasonably accommodated, up to the point of undue hardship, in accordance with the AHS Workplace Accommodation Policy.
Q3: What does it mean to be fully immunized?
You are considered fully immunized 14 days after the second dose of a two-dose vaccine series; or one dose of a one-dose vaccine series valid by Alberta Health.

- Book online with AHS or pharmacies.
- Call Health Link at 811.
- Contact a Physician Clinic.
