Your Opportunity:
As a Community Liaison Worker (CLW), you will provide services to clients seeking Mental Health assistance. You will provide one-to-one support to a diverse client population in a variety of Mental Health settings according to the client's individual service plan, and/or treatment plan in accordance with the client-centered approach. You will participate in assessments to determine access barriers and the support needs for clients accessing Mental health services. You will focus on assistance with collaboration and service coordination with additional support/resource modeling in a variety of settings. Completion of bachelor's degree in a health related field. Active or eligible for registration with applicable professional association or college. Minimum two years' experience working within Mental Health. Current Basic Cardiac Life Support - Health Care Provider (BCLS - HCP). Possession of a valid driver's licence and access to a motor vehicle in good repair for work-related travel.
